Everything you need for a smooth first visit — what to expect, what to bring, and our policies in plain language.

What to expect

Your first visit is longer than a typical doctor's appointment — and deliberately so. This is where we build your baseline.

  • A full health history review — your medical background, medications, family history, and health goals
  • A comprehensive physical exam tailored to your age and risk factors
  • Baseline lab work as appropriate, reviewed with you in plain language
  • A prevention plan — screenings, vaccinations, and next steps for the year ahead
  • Time for your questions — bring a list; we mean it

Plan on 45–60 minutes. Follow-up visits are typically 20–30 minutes. We don't overbook, so your time is genuinely yours.

What to bring

  • Photo ID (driver's license or passport)
  • Insurance card (if using insurance)
  • Complete medication list — names, doses, and how often you take each (a photo of the bottles works)
  • Prior medical records, if available (see below)
  • Vaccination records, if you have them
  • A list of your questions or concerns

Transferring your records

Coming from another practice? We'll help you move your records over — it's simpler than most people expect.

  1. Sign a release form — we'll provide one at your first visit (or send it ahead).
  2. We request them directly — from your previous PCP and any specialists.
  3. We review before you arrive — so your first visit builds on your history instead of starting from zero.

Records typically arrive within 1–2 weeks. If your first visit is sooner, bring what you have — we'll fill in the rest.

Before your visit

  • Complete any intake forms we send (via our secure patient portal)
  • Verify your insurance eligibility — we'll confirm, but a quick check on your plan's website helps avoid surprises
  • Arrive 15 minutes early for check-in
  • Bring your medication list and ID

Cancellation & no-show policy

Plain language, no fine print. Your appointment time is reserved specifically for you.

Need to cancel or reschedule?

Life happens. Please give us at least 24 hours' notice so we can offer your slot to another patient who needs it. Call or message us through the patient portal.

Late cancellations & no-shows

Cancellations with less than 24 hours' notice, or missed appointments without notice, may be subject to a fee. We'll always share the current fee schedule before your visit — no surprises.

Running late?

Call us if you're running behind. We'll do our best to accommodate you, but arrivals more than 15 minutes late may need to be rescheduled so we don't shortchange the patients after you.
